Sports Physical

If your child is planning on participating in the sports program at school, please make sure that they have the correct paperwork in place. All 7th and 8th grade children that are interested in participating in a sport this coming school year, the sports physical packet must be completed. This packet must be filled out completely and signed by a parent or guardian in the appropriate places or it will not be accepted. These forms are available on the district web-site under the Athletics tab. Please have your child bring the completed packet with them to the physical. Private physicals may be turned in to the trainer at CDHS. Children that have played a sport in the current school year need to have the Re-certification packet completed. They also still need to go to the physical to get re-certified. ALL students will need to have a pre-physical. Pre-physicals will be completed prior to your child seeing the doctor for certification.

If your child has asthma, diabetes or requires the use of an Epi-pen, please make sure that you fill out the appropriate paperwork for them to carry and self-administer their medication. These need to be renewed each school year. Sports forms and medication permission forms are available on the district web-site under Athletics.
